• (C) You have the right to know the name, qualifications, and credentials of all healthcare providers who participate in your treatment plan.
• (D) You have the right to receive a written description of the program prior to enrollment, including expected outcomes, potential risks, and any requirements for participation.
• (E) Any claim regarding the success of a weight-loss program must be substantiated and representative. Individual results vary based on adherence to treatment protocols and individual health factors.
